These are forms for internal use by faculty, staff and students of the UK School of Music. All other use is explicitly prohibited.

General Forms

School of Music Office Procedures (09-08-2005) MS Word.

Event Publicity Form (SOM web form) Use this to provide details about a concert or recital to Marketing to be included in the SoM web calendar, UK calendars and the media.

Photo Release Form (UK PDF Form) This is the official UK form which is REQUIRED to be filed for ANYONE (students, faculty, staff, or anyone esle) in order to use photo images taken by UK. If anyone in your ensemble has not filed a release form, we cannot use pictures of that ensemble in which individuals may be identifiable.

Friends of Music Financial Assistance Request (02-04-2008) PDF. Use this form to request financial assistance from the Friends for special projects.

Two-sided Recital Program Template (02-03-2006) MS Word. This is a half-sheet program (to be printed front and back and cut in half -- you get two programs per sheet of paper).

Four-page Recital Program Template (02-03-2006) MS Word. This is the more typical full-sheet program to be printed front and back and folded in half (you get one program per sheet of paper).
